Pericardial effusion / 심낭수

심장을 쌓고 있는 주머니인 심낭내에 혈청, 혈액 또는 고름이 쌓여 심장을 압박하는 질환으로 개에서는 비교적 흔한 질환 상태이다. 증가한 심장수에 의해 심장이 압박을 받아 심장의 박출량을 제한하여 급사 등 급격한 심장 기능저하를 유발할 수 있는 위험한 상태이다.

• 우심부전에 의한 울혈성 심부전

• 만성 신부전에 의한 요독증
• 출혈 / 신생물(종양)

• 갑상선 기능 저하증
• 독소 - 독소루비신 , 헤파린 , 흉부 방사선 치료
• 혈종 형성을 동반 한 심낭 외상 , 이물 이동 (예 : 이쑤시게) , pacemaker

임상 적으로 기력소실 식욕부진, 호흡부전, 또는 간헐적인 고혈등을 보일 수 있다. 경정맥의 박동성이 일반적인 소견중의 하나이다. 

청진상에서 심장의 박동음이 잘 청취되지 않을 수 있으며 심박의 이상 청취음이 빠르고 불규칙한 심장 박동에 따라 들릴 수 있다. 보통 우심부전에 의해 복수가 차오를 수 있다. 

진단은 방사선 검사 및 초음파 검사로 확진할 수 있으며 방사선 검사상 심장이 둥근 모양을 보이며 변연이 날카롭게 구분되는 특징을 보일 수 있고 폐 실질의 밀도 증가 소견이 보이며 때때로 흉수 및 기관지 공기조영상이 관찰될 수 있다. 

혈청 크레아티닌 키나아제 (0.3 ~ 2.9 U / L)와 심장 troponin 1 (기준 범위 : 0.0 ~ 0.7 ng / mL)의 현저한 상승이 특징적으로 나타날 수 있다. 

원인에 따라 이차적인 심장 압전, 심낭염, 폐부종, 울혈성 심부전이 나타날 수 있다. 

치료는 일반적으로 furosemide 또는 초음파 유도하 심낭수 배액이 추천된다. 보통 초음파 유도하 심낭수 배액은 개의 오른쪽 흉벽을 통해 16~18게이지의 카테터를 이용하여 실시될 수 있다.  

종양등에 의한 재발 성 심낭 삼출의 경우 심낭절제술을 통해 심낭에 저류액이 차는것을 흉수로 전환하는 수술적 요법이 추천될 수 있다. 신생물에 의한 심낭수 환자의 경우 예후가 불량하지만 양성 특발성 심낭수의 경우 예후가 양호한 편이다.
